2025-2026 Faith Formation Registration Forms

Moving forward, we will shut off the registration form on Thursday at midnight. This give us ample time to update rosters and make sure the teachers have an accurate count on Sunday morning. The registration form will re-open the following Monday afterwards. Thank you for your understanding!

***Money will not longer be accepted on Sunday mornings. This takes away from time needed to assist with Teachers needs and to get ready for the arrival of students. Money can be dropped off anytime in the drop box outside the front office, as well as in person Monday – Thursday between 9am – 4pm. Thank you for your understanding!

Sacramental Preparation

Reconciliation and First Holy Communion

  • Sacramental Preparation is a separate program outside of the normal religious education program. (K-12) This program has its separate registration form and fees associated with it.

Confirmation

  • Confirmation Preparation is a separate program outside of the Life Teen program.  This program has its separate registration form and fees associated with it.

Tuition Fee

The tuition cost for 2025-2026 are as follows:

Elementary Education and EDGE is $80 per child.

Tuition for LIFE TEEN is $100 per child.

These fees cover the cost of textbooks, class supplies, utilities, building and grounds maintenance, equipment usage, and salaries.

Tuition Assistance is available for qualifying families. Please call the Faith Formation office for information or with other tuition questions.

***Online Payments will not be utilized again this year for Tuition Costs. Please pay with Check or come with the exact amount in cash as we do not have a large supply of change to pull from. Thank you***

Faith Formation Class Sections

Please choose the class session or sessions your children will be attending this year.
Elementary: Sun AM 9:45-10:45
Edge (Middle School) Sun PM 12:00-1:30
Life Teen (High School) Sun PM 6:30-8:00

Photo Release

Throughout the religious education year, photographs or videos may be taken, and possibly used for publications in the Church Bulletin, on bulletin boards, and in slideshows on the TV’s in the Parish Hall and front office.

I give my consent for my child(ren) to be photographed during 2025-2026 Religious Education year.
